WEB Picnic Form

Hello Parent/Guardian, Our WEB (Where Everyone Belongs) program would like to take your 6th grader on a picnic to Soundview Park upper field adjacent to our gym on Wednesday, September 24 during lunch and 3rd period. WEB leaders will walk their group of 6th graders over to the field to have lunch and play games. Several staff members will also go along. We will head back to school at noon so students will be ready for 4th period.

*If you grant permission and you’re not updating your contact information or requesting a lunch, you do not need to return this form*

Who: All 6th graders and 8th grade WEB Leaders

What: A picnic! Either pack a lunch or they will get to go to the cafeteria to get lunch.

Where: Soundview Park, map below

When: Wednesday, September 24 from 11:00 a.m. – 12:15 p.m.

Why: For 6th graders to check in with their WEB leaders for support and FUN!
