FinalForms Online Registration

Whitman Middle School Athletics is using FinalForms, an online system, that boosts compliance and safety. FinalForms allows you to complete and sign forms from your computer, tablet,  or mobile device. FinalForms saves data from season-to-season and year-to-year, meaning you will never need to enter the same information twice! You may log in any time to update information including emergency medical contacts and information.


Associated Student Body (ASB) Request

We are excited to announce that the $25 WMS ASB (Associated Student Body) request will be collected at the start of the 2025-26 school year via the Source/SchoolPay. This request supports programs and activities that enrich your child’s school experience and help build a vibrant school community. Note: Students participating in sports are required to have and ASB sticker.     • Whitman may also offer a cross country club sport (co-ed) which also needs a Sports Physical

Athletic Registration

We are using FinalForms for online athletic registration. If you previously had an account, you can log in and all your info will still be there!

If you have not created an account, please follow the instructions below. A parent/guardian must create their FinalForms account first, before the student is able to create their account.

  1. Parents/guardians, go to the FinalForms website
  2. Type your name, date of birth, and email and then click register
  3. Check your email for a FinalForms email, and click confirm your account in the email text.
    • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com.
  4. Create your new FinalForms password and click confirm account.
  5. Click register student for your first child. Add your student and their email.

Physicals are valid for 24 consecutive months

  • The History document (Page 1) should be completed and remain with your provider
  • The Exam document (Page 2) should be completed and remain with your provider
  • The Medical Eligibility document (Page 3) should be the only item submitted to your school

Student-athletes must have health insurance to play a sport. If not covered by a parent/guardian’s insurance plan, insurance can be purchased through SPS.

Digital Tickets for High School Games

SPS home games (matches played at SPS high schools) can be purchased online via SPS HomeTown Ticketing Box Office.

This includes all regular season games for football and basketball. It also includes post-season games for volleyball, basketball, wrestling, gymnastics, baseball, soccer, softball (slowpitch and fastpitch), and track/field. Learn more about SPS Digital Athletic tickets.

Please note: tickets are posted under the HOST school (For example, West Seattle High School hosting Garfield High School would appear under West Seattle.)
