NOTE! Games sometimes can change last minute. If a change happens the ACs/Coaches will send an email out to parents.
Sports Paperwork & ASB Fees Due
All students need a doctor-signed physical uploaded to Final Forms before they can participate in sports. All forms must be submitted in Final Forms by the start date. Parents need to upload a current signed physical to Final Forms.
WMS Final Forms & ASB Fee Information.- Please make sure you register in Final Forms and upload a current, signed by a doctor, sports physical (sports physicals are good for 2 years from exam date). Sports Physical Participation Form
- All forms must be submitted in FinalForms by the start date.
If families are unable to access the Final Forms Online platform, they should contact their school athletic coordinator and/or athletic support staff at their building. Computers, as well as physical packets for uploads, can be made available for families to complete registration online.
