    Athletic Registration Required Forms:

    Student-Athlete Registration Form

    Pre-Participation Physical - History, Examination, and Clearance Form

    • Physicals must be completed & signed by a doctor. Physicals are valid for two years. Check with the Athletic Director to see if you have a valid one on file.

    Health Physical requirements:

    • 1st Physical - a New Athlete must have a Physical on file with the school nurse before being allowed to participate in any capacity.
    • 2nd Physical - is required every 2 years. A returning Athlete has to check their on file "Physical" expiration date in the front office before tryouts/practices begin.

    Concussion Form, & Parent/Guardian Release Form -
    All Athletes are to have a completed & sign the Concussion Form & Parent/Guardian Release Form by their parents/guardian in the front office within 1st week of making the Team.


    Academic Requirements:

    Weekly Progress Report - are required to be put in the Athletic Directors mailbox/classroom every Thursday, end of the day, prior to the scheduled event. Game Rosters will be issued to the coaches on Friday. No changes will be allowed after that time

    GPA Requirements-

    • Athletes need to maintain a 2.0 average GPA, with no more than one class having a grade of a D or E.
    • Athletes will receive a one week probation period for any failed effort/citizenship grades. If an Athlete fails to improve by the following week, (s)he will not be allowed to participate.

    Practice Gear

    Appropriate sportswear, sweatpants running shoes and a filled water bottle.

    General Requirements

    • Athletes cannot be under any disciplinary action on game day.
    • A player must participate in at least a of regular season meet to be eligible for Prelims/Metro participation.
